Tandem t:slim X2 insulin pumps: Malfunction 16
NZMS Diabetes has issued a product alert for the Tandem t:slim X2 insulin pump.
What's happening?
The maker of the Tandem t:slim X2 insulin pumps has identified a problem with some pumps which may have a higher rate of "malfunction 16" events. The malfunction appears as (Malfunction Code: 160x20EG) and is related to the speaker.
NZMS Diabetes has written to people with affected pumps and health care professionals.
What's the risk?
A Malfunction 16 will stop insulin delivery. Communication between the pump, CGM, and mobile app will also stop. If not addressed, this could result in hyperglycaemia.
What should I do?
If you have not experienced a Malfunction 16, continue using your Tandem pump as described in the User Guide Please take extra precautions because Malfunction 16 can occur at any time:
- Please use the Tandem t:slim mobile app with push notifications turned on so if the malfunction does occur, you will be notified by the app. Contact NZMS Technical Support if you need help turning on push notifications.
- If you are using a Dexcom sensor, please use the Dexcom mobile app as a backup for CGM alerts and alarms.
If you receive a Malfunction 16 alert:
- Promptly acknowledge it by pressing "Silence Alarm".
- Contact NZMS Technical Support by calling 0508 634 103 to report the malfunction and arrange replacement of the affected device.
- Switch to your backup method of insulin delivery as directed by your doctor or health care professional.
Regularly check your blood sugar to ensure you are not having unexpectedly high or low readings. Be prepared with a backup method of insulin delivery as directed by your doctor or health care professional, as set out in the pump training and in the pump user guide.
What is Tandem doing to fix the problem?
Tandem Diabetes Care will be releasing a software update aimed at enhancing early detection of speaker failure. This update will also introduce persistent vibration alerts to help reduce potential safety risk.
NZMS (the New Zealand supplier) will notify affected pump users when the software update becomes available.
