How to complete a bulk supply order (BSO)

This information is for ambulance providers who want to order repacked medicines from pharmacies after 1 July 2026.

Ambulance providers can use the BSO to have medicines repacked by a pharmacy into a smaller pack size. 

What should I write on the form?

Leave the top boxes and quantity on hand boxes empty.

In the table, include:

  • the date
  • the number and size of packs you want for each medicine, for example 10 x 20 tab
  • the name of the chemical and its presentation, for example:
    • Ticagrelor 90 mg tablets
    • Metoprolol tartrate 50 mg tablets
    • Prednisone 20 mg tablets

Who can sign the form?

The signature of an authorised representative of an ambulance service provider is sufficient. Include your ambulance provider organisation in the "name of private hospital or approved institution" box. 

How much can I order?

There is no volume limit when ordering on BSO, other than the expectation that no more than a reasonable amount for a month's use under usual circumstances would be ordered

Do I have to use a BSO form? 

Yes. Orders must be on a form supplied or approved by the Ministry of Health in order for the medicines to be funded. These orders must be on a BSO form.

How do I order repacks of controlled drugs on a BSO? 

Repacks of class C controlled drugs (e.g. codeine phosphate, tramadol) can be ordered using the standard BSO form that is used for repacks of other ambulance medicines.

There are no repacks of Class B controlled drugs in Section E.

Ambulance service providers can request the BSO form by e-mailing: customerservice@health.govt.nz

Small orders, more frequently are appreciated. Orders are limited to a maximum of five pads (of 20 forms) at a time. 

Allow 10 working days to process the order. The forms will come in the post from Bluestar.
