31 August 2009 Dear Pharmacist Subsidised Nicotine Replacement Therapy (NRT) – error in flow chart As you are aware, from 1 September 2009 NRT will be funded on the presentation of either a prescription or a Quit Card. This enables Practitioners, but not Quit Card Providers, to write a prescription for subsidised NRT as an alternative to the Quit Cards. Information Pack to pharmacy An information pack was sent to you last week to assist you with the implementation. An error with the registration numbers has been found in the second page of the summary chart. The registration numbers to be used for Quit Card providers and Quitline have been transcribed incorrectly. Please not that the registration numbers are correct on page one. They should read as follows: · MC88888 – Quit Cards issued by Quitline · MC99999 – Quit Cards issued by Community Quit Card Providers Columns two and three from the flow chart are below. Updated information is in bold. After 1 September 2009 From 1 December 2009

• Process - as prescription using A4 Code • Registration number – required - MC88888 for Quitline - MC99999 for Quit Card providers • Quit Card number – NOT required • Course - generally eight weeks via TWO Quit Cards • Dispense - four week course per Quit Card • Co-payment - Standard A4 co-payment $3 per item per Quit Card ($6 per item per course)

Process all Old Quit Cards as A4 from 1 September to 30 November

• Old Quit Cards are invalid – do not accept them
