5 September 2008 Dear Pharmaceutical Suppliers 2009 PTAC meetings and funding application timelines Please find below the 2009 dates for the Pharmacology and Therapeutics Advisory Committee (PTAC) meetings, and the last possible dates for receipt of funding applications that allow an application to be considered for inclusion in the agenda.

Meeting Date: Thursday 19 February Thursday 7 May Thursday 13 August Thursday 12 November

Applications to PHARMAC before: Tuesday 9 December 2008 Tuesday 10 March 2009 Tuesday 16 June Tuesday 15 September

When submitting a funding application, please address it to: Manager, Funding & Procurement PHARMAC Level 9 Cigna House 40 Mercer Street WELLINGTON When making a funding application, PHARMAC requires 15 spiral bound copies of the application, synopsis and supporting references. A CD of the full application, synopsis and supporting references and a sample of the pharmaceutical (if the application is for funding of a new pharmaceutical or new brand of pharmaceutical) are also required.
